This Document is intended to act as an aid to the completion of the Gambling
Commission’s ‘Annex A application’. Please Note: this is a guide only and has not
been endorsed by the Gambling Commission.
All applications from potential operators to the Gambling Commission are required
to complete the same online application form for operators. Named officers are
required to have either Personal Management Licences or in the case of Small
Scale Operators be Annexed to the main operating licence.
Questions need to be answered from the perspective of the individual not the
organisation.
NB part of the application requires the completion of a Disclosure and Barring
Check form. This needs to be initiated by the Gambling Commission so wait until
you receive the form rather than completing any held by your organisation.

It is critically important that you choose to have your identity confirmed by the
Gambling Commission – Although it looks like it’s possible for the employer to
confirm the identity the Gambling Commission have yet to verify the organisation so
this option isn’t actually available.
Physical Documents may be required (see this link.) At the time of writing; due to
COVID the Gambling Commission may accept digital copies over email but it
should be assumed physical documents are required in the first instance.
NB - 3 documents must be seen: One document from Group 1 plus any two from
Groups 1, 2a or 2b

note for many they need to be within the last three months
Further guidance from the Gambling Commission on the sending of
documentation:
1. When an application is submitted, the first thing we do is verify if all required
documentation has been provided
2. In an instance when someone has stated in the application form that they
wish for the Commission to verify their identity, we will then assess if this was
sent in to our office
3. If the documents were already sent in, we will just make a relevant note on
our system to say which documents were provided and then we would carry
on with the application processing
4. If we asses that these documents were not sent it, we will mark the
application as incomplete and send an information request to the applicant
requesting this information. After the request is made, the applicant will have
7 days to send the documents in
5. If the applicant doesn’t make the initial move to send these documents in,
they will be prompted by the Commission to do so
6. On the main PL Apply tool, when you submit an application, you will be
presented with the information as to where these documents should be sent
to
7. Due to Covid-19 restrictions that are being re-introduced the option to send
in the original documents might be temporarily suspended in the future so it’s
important for applicants to regularly review information available on our
website and on the application portal
